Businesses Deploying AI Agents Faster Than Safety Protocols Can Keep Up, Deloitte Warns

A stark new report from global consultancy giant Deloitte has issued an urgent warning: companies are racing to deploy AI agents at breakneck speed, but their safety protocols and governance frameworks are struggling to keep pace. The result? A growing wave of security, data privacy, and accountability risks that could derail the very promise of agentic AI.

According to Deloitte’s latest survey, agentic systems—AI agents capable of autonomous decision-making—are moving from pilot projects to full-scale production so rapidly that traditional risk controls, built for human-centred operations, are buckling under the pressure. The numbers are eye-opening: only 21% of organisations have implemented stringent governance or oversight for AI agents, even as adoption rates skyrocket. Currently, 23% of businesses are using AI agents, but that figure is expected to surge to 74% within the next two years. Meanwhile, the share of companies yet to adopt this technology is projected to plummet from 25% to a mere 5% over the same period.

Poor Governance is the Real Threat

Deloitte isn’t sounding the alarm because AI agents are inherently dangerous. Instead, the consultancy highlights that the real risks stem from poor context and weak governance. If agents operate as autonomous entities, their decisions and actions can quickly become opaque—making them difficult to manage and nearly impossible to insure against when things go wrong.

Ali Sarrafi, CEO and Founder of Kovant, a leading AI governance firm, argues that the solution lies in “governed autonomy.” “Well-designed agents with clear boundaries, policies, and definitions—managed the same way an enterprise manages any worker—can move fast on low-risk work inside clear guardrails, but escalate to humans when actions cross defined risk thresholds,” Sarrafi explains.

He continues, “With detailed action logs, observability, and human gatekeeping for high-impact decisions, agents stop being mysterious bots and become systems you can inspect, audit, and trust.” In other words, the companies that deploy AI agents with visibility and control will have the upper hand—not those who rush to implement them first.

Why AI Agents Require Robust Guardrails

AI agents may perform admirably in controlled demos, but they often stumble in real-world business environments where systems are fragmented and data is inconsistent. Sarrafi points to the unpredictable nature of AI agents in these scenarios. “When an agent is given too much context or scope at once, it becomes prone to hallucinations and unpredictable behavior,” he warns.

The answer, he says, is to limit the decision and context scope that models work with. “Production-grade systems decompose operations into narrower, focused tasks for individual agents, making behavior more predictable and easier to control. This structure also enables traceability and intervention, so failures can be detected early and escalated appropriately rather than causing cascading errors.”

Accountability for Insurable AI

As AI agents take real actions within business systems, accountability takes on a new dimension. With every action logged, agents’ activities become transparent and evaluable, allowing organizations to inspect actions in detail. This level of transparency is crucial for insurers, who are often reluctant to cover opaque AI systems.

Sarrafi emphasizes, “Such transparency helps insurers understand what agents have done and the controls involved, making it easier to assess risk. With human oversight for risk-critical actions and auditable, replayable workflows, organizations can produce systems that are more manageable for risk assessment.”

AAIF Standards: A Good First Step, But Not Enough

Shared standards, like those being developed by the Agentic AI Foundation (AAIF), are helping businesses integrate different agent systems. However, Sarrafi notes that current standardization efforts focus on what’s easiest to build, not what larger organizations need to operate agentic systems safely.

“Enterprises require standards that support operation control, including access permissions, approval workflows for high-impact actions, and auditable logs and observability,” Sarrafi says. “This way, teams can monitor behavior, investigate incidents, and prove compliance.”

Identity and Permissions: The First Line of Defense

Limiting what AI agents can access and the actions they can perform is critical to ensuring safety in real business environments. Sarrafi warns, “When agents are given broad privileges or too much context, they become unpredictable and pose security or compliance risks.”

Visibility and monitoring are essential to keep agents operating within limits. “Only then can stakeholders have confidence in the adoption of the technology,” Sarrafi adds. “If every action is logged and manageable, teams can see what has happened, identify issues, and better understand why events occurred.”

He concludes, “This visibility, combined with human supervision where it matters, turns AI agents from inscrutable components into systems that can be inspected, replayed, and audited. It also allows rapid investigation and correction when issues arise, which boosts trust among operators, risk teams, and insurers alike.”

Deloitte’s Blueprint for Safe AI Agent Governance

Deloitte’s strategy for safe AI agent governance sets out defined boundaries for the decisions agentic systems can make. For instance, they might operate with tiered autonomy—starting with agents that can only view information or offer suggestions, then progressing to limited actions with human approval, and finally allowing automatic actions once reliability is proven in low-risk areas.

Deloitte’s “Cyber AI Blueprints” suggest embedding governance layers, policies, and compliance capability roadmaps into organizational controls. Ultimately, governance structures that track AI use and risk, and embedding oversight into daily operations, are essential for safe agentic AI use.

Readying workforces with training is another critical aspect of safe governance. Deloitte recommends training employees on what they shouldn’t share with AI systems, what to do if agents go off track, and how to spot unusual, potentially dangerous behavior. If employees fail to understand how AI systems work and their potential risks, they may unintentionally weaken security controls.

Robust governance and control, alongside shared literacy, are fundamental to the safe deployment and operation of AI agents, enabling secure, compliant, and accountable performance in real-world environments.
